Bridge Home Health and Hospice in San Diego, CA seeks a part-time Per Diem Physical Therapist Assistant for home health visits. The PTA provides direct therapy under the direction of a licensed physical therapist and follows the physician's plan of care to support patient goals.

Responsibilities include delivering therapeutic exercises, using modalities, documenting progress, coordinating with team, and ensuring patient safety.

Qualifications

• Graduate of an accredited PTA program with an Associate's degree approved by APTA
• Approved as PTA by the California Physical Therapy Board
• Good verbal and written communication and organization skills
• CPR certification current
• Valid California driver's license with insured vehicle

Responsabilités

• Provides direct physical therapy according to directions of a qualified physical therapist and in accordance with the physician's plan of care
• Treats patient to relieve pain, develop or restore function, and maintain maximum performance
• Directs and aids patients in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities and prosthetic training
• Utilizes physical agents (heat, cold, water, light, electricity, ultrasound, massage)
• Observes and reports patient responses to treatment to the physical therapist and organization staff
• Instructs patients in care and use of wheelchairs, braces, crutches, canes, and prosthetic/orthotic devices
• Maintains necessary records and notes incorporated into the patient's clinical record
• Participates in Home Care Team meetings and Inservice Education Program
• Performs other duties as assigned
• Adheres to attendance and reliability guidelines to ensure quality patient care
