Job summary

Humana, Inc. through CenterWell Home Health is seeking a licensed Physical Therapist Assistant to deliver in-home care, helping patients regain strength and independence. You will work under the Physical Therapist to implement treatment plans in the patient’s home.

You will coordinate with a multidisciplinary team, document care, and participate in ongoing education while adhering to infection control practices and safety standards in a patient-facing, home-based setting.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatric population changes (physical, emotional, social, biological).
• Read, write, and speak fluent English.

Responsibilities

• Administer physical therapy treatment plan to patients, working onsite in their homes, as developed by the Physical Therapist.
• Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities to patients.
• Document client records and submit clinical documentation to the office on time each week.
• Attend educational meetings and coordinate department in‑services.
• Notify Physical Therapist or Branch Director of any equipment failures, lack of supplies, or staff or client changes.
• Apply concepts of infection control and universal precautions in performing patient care activities.
