Job summary

Halcyon Home LLC in Austin, Texas is seeking a Home Health Physical Therapist Assistant (PTA) PRN to support geriatric patients in the Northeast Austin area and surrounding communities. A minimum of one year of home health experience is preferred, with knowledge of Homecare Homebase EMR considered a plus.

As a PTA, you will provide compassionate physical therapy in both community and residential settings, collaborating with physicians and caregivers to implement and adjust the plan of care.

Qualifications

• Current PTA license in Texas.
• Experience in home health preferred.
• Strong communication and interpersonal skills.
• Ability to work independently in a fast-paced schedule.

Responsibilities

• Performs ongoing clinical assessment and visits to identify patient needs.
• Develops and evaluates the plan of care with patient and caregiver.
• Documents clinical notes and revises POC as needed.
• Supervises and guides assistants and aides when appropriate.
• Provides treatment for neuromuscular, neurological, and orthopedic conditions.
