Halcyon Home LLC is seeking a charismatic Home Health Physical Therapist Assistant (PTA) to join our fast-growing geriatric home health team in Northeast Austin and nearby communities. This PRN position supports flexible scheduling with a strong home health background preferred.

Current PTA license required; Homecare Homebase EMR knowledge is a plus. You will assess patients, develop care plans, educate families, and coordinate with physicians while delivering compassionate patient care in

Qualifications

Current PTA license.
Knowledge or experience using Homecare Homebase EMR is a plus.
Experience in home health or related healthcare setting preferred.
Strong clinical and assessment skills.
Excellent communication and interpersonal abilities.
Ability to work independently and manage time effectively.

Responsibilities

Performs ongoing clinical assessment including OASIS at appropriate time points.
Develops and evaluates the plan of care with patient and caregiver(s).
Documents notes, orders, and records in a timely manner; revises POC based on data.
Accepts assignments within education and competence.
Provides supervision to PTAs and home health aides as appropriate.
Plans and provides treatment for neuromuscular, neurological, and orthopedic disorders.
Communicates with physicians and other health care practitioners about the plan of care.
Meets continuing education requirements.
Instructs patient and family in transfer techniques and use of equipment.
Provides patient and caregiver education and counseling.
Oversees team members where applicable and follows infection control protocols.

As a Physical Therapist Assistant, you will provide physical therapy to our patients in both community and residential settings in the Northeast Austin area and surrounding areas. A minimum of one year of home health experience is preferred, along with a proven ability to manage a fast-paced, independent work schedule. Knowledge of Homecare Homebase software is a plus!

Essential Functions:

Performs ongoing clinical assessment, including OASIS assessments, at appropriate time points to identify patient needs, determine level of functioning, and appropriateness of patient for home care.
Develops and evaluates the plan of care in partnership with the patient, representative (if any), and caregiver(s).
Documents, prepares, and completes clinical notes, phone orders, and other clinical record documentation on an ongoing basis and in a timely manner. Revises the POC based on evaluation and ongoing assessment data.
Accepts clinical assignments that are consistent with education and competence to care for patients.
Provides supervision and guidance to physical therapy assistants and home health aides, as appropriate.
Plans and provides appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessment.
Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.
Meets mandatory continuing education requirements of the Agency and licensing board.
Assists and instructs patients and families in transfer techniques appropriate to patient abilities. Instructs on use of equipment such as walkers, wheelchair, canes, etc., in a safe manner. Utilizes appropriate therapy equipment specific to patients’ diagnoses and needs.
Provides patient, caregiver, and family counseling; Provides patient and caregiver education.
Provides services that are ordered by the physician as indicated in the plan of care; oversees other team members, when applicable, in the performance of patient care that is consistent with patients’ needs.
Applies concepts of infection control and standard precautions in coordinating or performing patient care activities to prevent contamination and transmission of disease.
