Job summary

BAYADA Home Health Care, Inc. is seeking a compassionate Home Health Physical Therapy Assistant (PTA) for Avalon, NJ. This staff, per-visit position serves adult and geriatric clients in Cape May County with in-home care in designated territories.

Qualifications

• Licensed nurse with NJ credentials and PTA-related clinical experience.
• Minimum one year of recent, verifiable clinical experience in home health or related setting.
• Ability to travel to cases in designated geographic territories.
• Strong interpersonal and documentation skills to support patient care.

Responsibilities

• Follow a designated care plan in accordance with patient's needs.
• Make home visits to clients in designated geographic territories.
• Perform duties including administration of medication, wound care, treatments and procedures.
• Monitor clients' conditions and report changes to Clinical or Client Services Manager.
• Follow up with and document doctors' orders accurately.
• Perform client assessments as necessary and coordinate case management.
• Document observations and evaluations using a state-of-the-art touch pad tablet.
