Job summary

BAYADA Home Health Care, Inc. is seeking a compassionate Home Health PTA to support adult and geriatric clients in Cape May County on a per-visit basis. The PTA will visit clients in their homes, follow care plans, and document care using EMR systems. One year of clinical PTA experience and NJ licensure are required; home health experience is preferred.

Qualifications

• One year of clinical (medical/surgical) experience required.
• Licensed PTA in good standing in New Jersey.
• Home health experience preferred but not required.

Responsibilities

• Follow a designated care plan according to patient's needs.
• Make home visits to clients in designated geographic territories.
• Administer medications, wound care, treatments, and procedures as directed.
• Monitor client conditions and report changes to supervisor.
• Document doctors' orders and care observations accurately.
• Coordinate case management and ensure proper documentation.
