Job summary

Atrinity Home Health, part of LHC Group within the Optum family, is seeking a Physical Therapist Assistant to perform treatment, exercise instruction, and patient education under the care plan. The role focuses on home health interventions with a patient-centered approach.

Responsibilities include implementing the plan of care, instructing patients and caregivers, and coordinating with supervisors to adjust treatment as needed.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Available to work a weekend rotation.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Follow state-specific laws governing home care physical therapy and follow the plan of care.
• Instructs and aids patients in active and passive exercise, gait, ADL, and functional training.
• Observe, record, and report patient responses and coordinate care with the team.
