Job summary

Atrinity Home Health, part of LHC Group and a member of the UnitedHealth Group family, seeks a Physical Therapist to assess and plan care for patients in home health. This role focuses on compassionate care and meaningful connections with patients and families.

Responsibilities include evaluating functional status, developing and implementing treatment plans, documenting progress, and collaborating with the care team to optimize patient outcomes.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
• Ability to function in any home situation regardless of patient demographics

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists in developing the therapy plan of care.
• Provides services within the scope of practice and in accordance with the plan of care.
• Observes and reports patient response to treatment and changes in condition to the supervising nurse/physician.
• Treats patients to relieve pain and restore or maintain function.
• Instructs patient, family, and care team regarding plan of care and progress toward goals.
• Visits patients per Plan of Treatment and documents progress notes.
• Completes evaluations and develops the PT plan of care per state guidelines.
• Reports outcomes of evaluation, goals, and projected frequency of care
