Job summary

Mass General Brigham Home Care seeks a licensed Physical Therapy Assistant to deliver one-on-one in-home therapy across a defined service area in the Boston region. You will work under a Physical Therapist, implementing treatment plans, guiding exercises, and promoting safety and mobility for patients in their homes.

The role offers a 40-hour week with a hybrid work model and requires travel within Mass General Brigham Healthcare at Home’s geographic area.

Qualifications

• Two years' experience in acute care, rehab or homecare as a Physical Therapy Assistant.
• Licensed as a Physical Therapist Assistant in Massachusetts.
• Proficient computer skills and ability to learn other software applications.
• Strong verbal, written and electronic communications skills.
• Travel required within Mass General Brigham Healthcare at Home's geographic area.
• Ability to work with various computer software and handheld patient care devices.

Responsibilities

• Provide one-on-one home-based physical therapy under the direction of a Physical Therapist.
• Implement individualized treatment plans.
• Provide and guide therapeutic exercises to improve mobility, safety, and independence.
• Reinforce fall prevention and home safety strategies.
• Educate patients and caregivers on exercises and safe mobility techniques.
• Monitor progress and report changes to the supervising Physical Therapist.
• Document treatments and patient responses; collaborate with interdisciplinary care team.
