Job summary

Responsibilities

• Establishes therapy goals and formulates a treatment plan based on evaluation results.
• Supervises PTAs according to organization policy and state regulations.
• Provides individual, family and group therapy sessions for outpatients.
• Participates in interdisciplinary group for hospice patients and case conferences for home health patients.
• Participates in discharge planning.
• Assesses clinical needs using standardized tools and facilitates treatment recommendations.
• Records data in the patient's medical records.
