Choice Health At Home in Altamonte Springs is seeking a Licensed Physical Therapist Assistant to join our home health team. You will support licensed physical therapists by delivering targeted rehab plans, guiding patients through exercises, and monitoring progress to promote independence and safer daily living.

This full-time role emphasizes compassionate care, accurate documentation, and collaboration with families and caregivers to optimize recovery outcomes in a home setting.

Qualifications

• Successful completion of accredited physical therapy program and state licensure exam.
• Valid Physical Therapy Assistant license.
• 1 year of home health experience preferred.
• AHA CPR certification.
• Valid Driver's License and Auto Insurance.
• HomeCare HomeBase experience preferred.

Responsabilités

• Assist physical therapists in executing individualized treatment plans for patients with musculoskeletal and neurological conditions.
• Conduct therapeutic exercises, manual therapy techniques, and modalities such as ultrasound or electrical stimulation under the supervision of a physical therapist.
• Monitor and document patient responses to treatment, reporting progress to the supervising physical therapist.
• Educate patients and families on exercises, proper use of assistive devices, and strategies to prevent injury or re-injury.
• Maintain a safe and clean treatment environment, ensuring equipment is sanitized and in good working order.
