Job summary

Qualifications

• Successful completion of an accredited physical therapy program.
• Current state Physical Therapy license.
• CPR certification.
• 1 year of home health experience preferred.
• Valid driver’s license and auto insurance are required.

Responsibilities

• Assess, plan, organize and participate in rehabilitative programs.
• Perform and document an initial exam to identify problems and determine diagnosis.
• Plan and carry out individualized programs of physical treatment.
• Evaluate effects of treatment and adjust as needed for maximum benefit.
• Apply infection control/universal precautions in patient care.
