Job summary

Human Touch Home Care seeks a dedicated Physical Therapist Assistant to join our Colorado team. You will deliver patient-centered care under the supervision of a Physical Therapist, educating patients and families and documenting progress in a timely manner.

Qualifications

• PTA program accredited by CAPTE or equivalent.
• Currently licensed in the state(s) where practicing, or eligible to take the PTA examination.
• Two years of experience preferred.
• Ability to exercise initiative and independent judgment.

Responsibilities

• Performs services planned, delegated and supervised by the Physical Therapist.
• Assists in preparing clinical and progress notes.
• Educates patient and family in treatment plans and home programs.
• Coordinates therapy with PT and other staff.
• Documents care plans and progress notes timely per policy.
• Adheres to Agency policies and procedures.
