- Deliver home health PT services under licensed PT supervision per plan of care.
- Identify patient goals and implement mobility-focused interventions.
- Document services and maintain patient records.
- Develop individualized care plans and wellness programs.
- Educate and motivate patients for rehabilitation participation.
- Collaborate with other clinicians to ensure coordinated care.
- Monitor progress and report outcomes to supervising therapist.
