Job summary

Freedom at Home is seeking a compassionate Physical Therapist Assistant (PTA) to join our home health team in a full-time, part-time, or PRN capacity. You will provide skilled PT services in patients' homes under a licensed PT, following individualized treatment plans to improve mobility and independence.

Qualifications include graduation from an accredited PTA program, an active PTA license, CPR certification, and reliable transportation. Prior home health experience is a plus.

Qualifications

• Graduate of an accredited PTA program.
• Current Physical Therapist Assistant license in the state of practice.
• Current CPR certification.
• Minimum of two years of PTA experience preferred.
• Home Health experience is a plus but not required.
• Valid driver's license, reliable transportation, and auto insurance.

Responsibilities

• Provide skilled physical therapy services under the direction and supervision of a licensed Physical Therapist.
• Carry out individualized treatment plans designed to improve mobility, strength, balance, function, and overall quality of life.
• Assist patients with therapeutic exercises, gait training, transfer training, balance activities, and use of assistive devices.
• Monitor patient progress and communicate changes to the supervising Physical Therapist.
• Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques.
• Complete accurate, timely clinical documentation.
• Participate in case conferences, team meetings, and quality improvement initiatives.
• Collaborate with physicians, nurses, therapists, and other healthcare professionals to provide coordinated patient care.
• Promote safe, compassionate, high-quality care that helps patients remain independent in their homes.

Education

Graduate of an accredited PTA program

Current PTA license in state of practice

Current CPR certification
