Job summary

Interim HealthCare is seeking a Home Health Physical Therapist Assistant to provide in-home physical therapy to patients in Berks County, PA and surrounding areas. You will work with a multidisciplinary home health team to implement the plan of care, assist with exercises, and educate patients and families on self-care.

Qualifications

• Graduate of an accredited Physical Therapist Program.
• Active PTA license in PA.
• CPR certification required.

Responsibilities

• Provide home-based physical therapy to patients.
• Collaborate with home health team to implement plan of care.
• Assist patients with exercises and hands-on therapy to improve mobility.
• Educate patients and families on the plan of care and exercises.
• Support family and caregivers in safely assisting with exercises.
