Job summary

Interim HealthCare is hiring a Home Health Physical Therapist Assistant (PTA) to join our Janesville, WI team. In this in-home role you will help patients regain mobility and manage pain, delivering care that supports independence and quality of life.

You will work with a home health team to implement plans of care, document progress and educate patients and families. Flexible day shifts and a supportive, clinician-friendly environment are offered.

Qualifications

• Graduate of an accredited Physical Therapist Program.
• Active PTA license in Wisconsin.
• Minimum 1 year PTA experience, preferably in home health care.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Good clinical judgement, strong interpersonal skills, dexterity and compassion.

Responsibilities

• Provide home-based physical therapy to improve mobility and strength.
• Collaborate with home health team (RN, LPN, CNA, HHA, OT, SLP) to meet care plans.
• Observe movements, document progress and modify care plans as needed.
• Assist patients with exercises and hands-on therapy.
• Educate patients on their plan of care, goals and self-care.
