Interim HealthCare of Madison in Wisconsin seeks a qualified Physical Therapist Assistant (PTA) to join our home health team serving Janesville and surrounding areas. You will deliver one-on-one home-based physical therapy to patients recovering mobility after illness or injury.

The role emphasizes compassionate patient education, hands-on therapy, and collaboration with RNs, LPNs, and other clinicians to support each patient’s plan of care and goals.

Qualifications

• Graduate of an accredited Physical Therapist Program and active PTA license in Wisconsin.
• CPR certification.
• Minimum of 1 year PTA experience, ideally in home healthcare.
• Knowledge of state and federal home health regulations.

Responsibilities

• Provide home-based physical therapy to patients who have lost mobility, strength and function.
• Work as part of a home health team including RN, LPN, CNA, HHA, OT and SLP.
• Observe movements, conduct assessments, document progress and modify care plan as needed.
• Assist patients with exercises and hands-on therapy to improve mobility and reduce pain.
• Educate patients on plan of care, goals, exercises and self-care.
• Coach family on the plan of care and safely assisting with exercises.
