Job summary

Wesley Homes is seeking a dedicated Physical Therapist Assistant to provide hands-on support under a licensed Physical Therapist, improving mobility and independence for home health patients. You will promote safety and quality of movement in patients' home environments while supporting special caregiving needs and family involvement.

Under supervision, you will implement treatment plans, monitor progress, and document outcomes.

Qualifications

• Associate degree in Physical Therapist Assistant required.
• Current Washington State PTA license required.
• CPR & BLS certification required.

Responsibilities

• Implement physical therapy treatment plans under supervising PT.
• Provide therapeutic exercises, gait training, and mobility interventions at patients' homes.
• Monitor and document patient progress, reporting changes to the supervising PT.
• Collaborate with patients, families, and caregivers to promote active participation in care.
• Ensure proper use of equipment and educate patients and caregivers on devices.
