CenterWell Home Health is seeking a Physical Therapist Assistant to deliver patient-centered therapy in home settings under the supervision of a Physical Therapist. You will implement treatment plans, perform therapeutic exercises, and help patients regain strength and independence in their own homes.

Strong collaboration with healthcare teams, adherence to infection control, and timely documentation are essential. Valid PT license, CPR, and a driver's license are required for this role.

Competenze

• Current state license to practice Physical Therapy
• Valid driver’s license and reliable transportation
• Current CPR certification
• Experience collaborating with healthcare professionals
• Knowledge of geriatric physical, emotional, social, and biological changes
• Fluent English reading, writing and speaking

Mansioni

• Administer the physical therapy treatment plan to patients, working onsite in their homes, as developed by the Physical Therapist.
• Administer non-complex manual therapeutic exercises, massage, traction, heat, light, cold, water, and electrical modalities.
• Document client records and submit clinical documentation to the office on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment failures, supply shortages, or staff or client changes.
• Apply infection control and universal precautions in patient care activities.
