Job summary

Ecumen is seeking a Home Care Physical Therapy Assistant in Minnesota to support therapists in delivering patient rehabilitation during weekday days, 8am-4:30pm. The role focuses on improving mobility, function and strength in a caring, team-oriented environment.

Qualifications

• High school diploma or GED.
• Licensure/Certification as required by the state.
• Ability to communicate effectively in both verbal and written formats.
• Must be flexible and adaptable to changing situations including able to work irregular hours.

Responsibilities

• Performs therapeutic procedures by administering manual exercises; instructing, encouraging, and assisting patients in performing physical activities under the direction of the Physical Therapist.
• Instructs and counsels patients by describing therapeutic regimens and promoting wellness.
• Documents patient care services by recording progress during therapy sessions and tracking range of motion.
• Identifies patient care issues by interviewing patients and reviewing therapy history.
• Cleans and prepares equipment for therapy sessions and maintains supplies.
• Keeps equipment operating by following instructions and performing maintenance.
• Practices within state licensure requirements and regulations set by CMS and other payors.
• Manages and maintains license/certification renewals and other professional certifications.
