Job summary

LiveWell Homecare Agency, LLC in Birmingham, AL seeks a Physical Therapy Assistant (PTA) to join our home health team. You will provide direct PT services under a plan of care and collaborate with a physician and physical therapist to deliver high-quality care in patients' homes.

Ideal candidates hold an accredited PTA degree, an active state PTA license, and CPR certification. Experience in home health and OASIS is preferred.

Qualifications

• Graduation from an accredited PTA program.
• Current, unrestricted state PTA license.
• CPR certification required.

Responsibilities

• Provide direct physical therapy services based on plan of care.
• Assess patient conditions and deliver skilled treatments.
• Educate patients and caregivers on use of devices.
• Communicate with patients, families, and team members to optimize care.
• Document patient progress and treatment details accurately.
• Assist in discharge planning and provide ongoing care instructions.
• Adhere to ethical standards and maintain confidentiality.
