AHN Healthcare@Home in Erie is hiring a Physical Therapist Assistant (PTA) for a Home Health position serving patients in the Erie area. Eligible for a $5,000 first-year success bonus for achieving key milestones.

Qualifications

• Active Physical Therapist Assistant (PTA) license in the state you work.
• One year of PTA experience delivering patient care.
• Valid driver’s license and ability to spend ~20% of your workday driving to patient locations.

Responsibilities

• Provide compassionate, patient-centered Physical Therapy services in the home health setting under the direction of a licensed Physical Therapist.
• Support patients in restoring mobility, reducing pain, and maximizing functional independence through therapeutic exercises, gait training, transfer training, muscle re-education, and prosthetic or orthotic support.
• Educate patients and caregivers on the safe use of mobility and assistive devices, while monitoring progress and communicating patient responses and condition changes to the care team.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory
