Job summary

Adoration Home Health in Grand Junction, CO is hiring a Physical Therapist Assistant to provide 1:1 home health care under the supervision of a Physical Therapist. This full-time role includes a $5K sign-on bonus and a collaborative team environment.

You will assess patient needs, document care, and coordinate equipment with the Physical Therapist to enhance rehabilitation outcomes. We value compassionate, reliable clinicians who can work independently while staying connected with the team.

Qualifications

• Current unrestricted license as a Physical Therapist Assistant in the practicing state.
• Current Basic Life Support or CPR certification.
• A minimum of one year's work experience as a Physical Therapist Assistant preferred.
• Demonstrates knowledge of rehabilitation techniques related to complex neurological injury.

Responsibilities

• Provides physical therapy services planned, delegated and supervised by the qualified Physical Therapist according to the Plan of Treatment.
• Regularly communicates with the Physical Therapist regarding progress and questions.
• Communicates patient needs and progress to the Physical Therapist and assists in securing adaptive equipment to improve function.
• Maintains and submits patient treatment and billing documentation per regulations and timelines.
