Job summary

Graham Healthcare Group is seeking a Home Health Physical Therapy Assistant for a position in Evanston, Illinois. The role supports patients in Chicago, Lincolnwood and surrounding areas with a strong emphasis on team-based care and timely documentation.

A $5,000 first-year bonus rewards achievement of key milestones. Minimum qualifications include an accredited Associate degree in physical therapy, an active PTA license, and prior clinical experience.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Minimum one year of experience in acute care and/or rehabilitative settings preferred.
• Must hold license/registration as a Physical Therapist Assistant in good standing.
• Current driver's license and ability to travel ~20% of the day.
• Commitment to meet critical charting deadlines and manage time effectively.

Responsibilities

• Collaborate to provide management and delivery of patient care plans.
• Support scheduling and care coordination within a team-based Home Health environment.
• Demonstrate the skills to self-manage time and travel between patient locations.
• Utilize EMR systems and mobile devices to document care and progress.
