Job summary

Elara Caring is seeking a Physical Therapist for a home health role in Cherry Valley, MA. You will provide direct patient care, develop individualized plans, and collaborate with a multidisciplinary team to improve outcomes in patients' homes.

Ideal candidates have current MA PT licensure, at least 1 year of healthcare experience, and the ability to travel within Worcester and surrounding areas (50% travel). A sign-on bonus and comprehensive benefits are available.

Qualifications

• Graduate of an APTA-approved Physical Therapy program.
• Current, unrestricted PT license in Massachusetts.
• Minimum 1 year of healthcare experience.
• Ability to travel within Worcester and surrounding areas (50% travel).
• Reliable vehicle and valid driver’s license.
• Ability to lift 50–100 lbs and perform full range of body motion.

Responsibilities

• Provide direct patient care, evaluate functional needs, and design individualized care plans
• Collaborate with other healthcare professionals to ensure optimal outcomes
• Document evaluations, treatment goals, and progress
• Promote physical therapy in the community
• Work in a physically demanding, high-energy environment
