Job summary

Redcrest Home Health LLC provides comprehensive skilled physical therapy in patients' homes across Bloomington and surrounding communities. The PT educates families, ensures safety, and documents care per physician orders.

The role requires at least one year in acute PT, home health experience preferred, and a valid driver’s license. Expect dedicated on‑call coverage and adherence to agency and Medicare guidelines.

Qualifications

• Minimum of one year of experience as a PT in an acute setting.
• Home care experience is desired.
• Ability to effectively communicate with patients, families, physicians and co-workers.
• Knowledgeable on therapy best practices.
• Ability to make appropriate physical therapy judgments.
• Valid driver’s license and auto insurance in your name.

Responsibilities

• Provide skilled physical therapy in patients' homes as ordered by the physician.
• Observe, document, and report patient progress to the attending physician and agency staff.
• Perform OASIS assessments and develop/adjust plans of care as needed.
• Instruct patients and families on use of equipment and home exercise programs.
• Ensure documentation complies with agency and Medicare guidelines.
