Job summary

Redcrest Home Health is seeking a Physical Therapist I to provide skilled home health PT services in Bloomington and surrounding communities. You will educate patients and families, complete documentation, and ensure safety during care.

The role requires active PT licensure and at least one year of acute PT experience, with on-call duties and a Monday–Friday schedule. A $10,000 sign-on bonus is offered.

Qualifications

• Minimum of one year of PT experience in an acute setting required.
• Home care experience desired.
• Ability to communicate effectively with patients, families, physicians and staff.
• Detail-oriented with strong assessment and documentation skills.

Responsibilities

• Provide skilled physical therapy in patients' homes as ordered by physician.
• Observe and report patient changes to physician and agency staff.
• Perform OASIS assessments and develop/adjust care plans.
• Educate patients and families on plan of care, equipment use, and safety.
• Supervise PTAs and aides when appropriate; complete documentation per guidelines.
• Maintain license and participate in on-call rotations and trainings.
