Job summary

UnitedHealth Group is hiring a Physical Therapist to provide home health services, performing assessments, developing and implementing plans of care, and collaborating with the care team. In this role you will evaluate functional status, plan interventions, monitor progress, and ensure compliance with state practice guidelines while delivering compassionate patient care in patients' homes.

Sign-on bonus of $5,000 is available for full-time availability; flexible scheduling and benefit-rich

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs in home health.
• Assists the physician in developing the therapy plan of care.
• Provides services within the scope of practice per state laws and plan of care.
• Observes and reports patient response to treatment to the supervising nurse/physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs and informs health care team, patient, and family/caregivers about plan of care and progress.
• Completes patient evaluations and develops PT plan of care per state guidelines.
• Reports outcomes and anticipated frequency of care.
