Job summary

UnitedHealth Group is seeking a Physical Therapist for home health to assess and address patient needs related to mobility and functional status. You will develop and implement a treatment plan, perform interventions to improve well-being, and monitor progress.

Responsibilities include evaluating functional status, providing PT services within state guidelines, and coordinating with the health care team. Sign-on bonus and flexible scheduling are offered as part of the compensation package.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and develops the PT plan of care.
• Provides services within the scope of practice as defined by state laws and plan of care.
• Observes and reports the patient's response to treatment and any changes in condition to the supervising nurse/physician.
• Instructs patient and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
• Completes patient evaluations and PT plan of care within state guidelines and submits progress notes.
