You will assess patients in their homes, develop treatment plans, and deliver interventions to improve mobility, endurance and comfort while documenting progress accurately. Responsibilities include performing evaluations, coordinating with physicians and nurses, and educating patients and families about care plans.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Valid driver's license and reliable transportation.
• Preferred CPR certification or ability to obtain within 90 days.

Responsibilities

• Evaluate patient status and assist in developing the plan of care.
• Provide PT interventions to improve function and reduce pain.
• Monitor patient progress and document outcomes per policy.
• Educate patients and families about plan of care and safety.
• Complete visits and progress notes according to schedule.
• Ensure care complies with state practice guidelines.
• Communicate patient changes to the care team promptly.
• Function effectively with diverse patients in home settings.
