Advocate Health at Home is seeking a full-time Home Health Physical Therapist to manage an in-home patient caseload in the University City area of Charlotte, NC. You will assess, plan, and deliver PT services per physician orders, coordinating with a multidisciplinary team and documenting progress in the EMR.

The role requires a DPT, stateLicensure, and BLS certification; prior home health experience is preferred.

Qualifikationen

• DPT required with licensure in the work state.
• BLS for Healthcare Provider certification preferred.
• 1+ year home health clinical experience preferred.

Aufgaben

• Manage daily patient caseload and ensure timely care per plan of care.
• Collect data from assessments, telephonic contacts, and EMR.
• Analyze data to identify risks and coordinate care to optimize outcomes.
• Prioritize clinical issues based on patient health and safety.
• Evaluate rehabilitation needs and develop plans of care with physician input.
• Coordinate multidisciplinary team and supervise licensed staff.
• Maintain communication with the care team and physician orders.
• Incorporate goals into therapy and document progress.
