Job summary

PARS Therapy is seeking a compassionate and skilled Physical Therapist to join our home health team in Winnie, TX. This role focuses on helping patients regain movement, reduce pain, and improve quality of life in the comfort of their homes.

You will deliver evidence-based care, tailor treatment plans, educate patients and caregivers, and collaborate with an interdisciplinary team to ensure safe, effective outcomes.

Qualifications

• Active Physical Therapist license in Texas (TX)
• Professional liability insurance coverage for TX PT license
• Graduate degree or higher in Physical Therapy from an accredited program
• Experience in home health or rehabilitative settings preferred
• Ability to adapt care approaches to varied patient needs
• Up-to-date knowledge of evidence-based treatment techniques and best practices
• Excellent communication skills and professional demeanor in patient education

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting
• Develop and implement individualized treatment plans to restore function, mobility and alleviate pain
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining
• Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies
• Monitor patient progress and adjust plans to achieve outcomes
• Accurately document evaluations, treatment sessions, and progress notes using digital platforms
• Collaborate with physicians, nurses, and occupational therapists
• Conduct home safety assessments and make fall-prevention recommendations
• Deliver compassionate care and empower patients toward independence
