Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist to deliver in-home care, working with the physician and care team to restore function with patient-centered goals.

You will assess, plan, and implement PT treatments, supervise aides, document progress, and communicate with patients and families. Requires PT degree, active license, CPR, and teamwork. Base pay ranges from $93,000 to $128,000 per year, with per-visit pay as applicable.

Qualifications

• Must have graduated from an accredited Physical Therapy program (APTA approved).
• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy in patients' homes to attain highest physical function.
• Assess and document patients' physical strengths to guide treatment goals.
• Develop and implement rehabilitation programs aligned with physician plans.
• Provide clear care instructions to PTAs and Home Health Aides and oversee their documentation.
• Document observations, interventions, and evaluations.
• Report progress to physician and care team, and submit required notes and discharge summaries.
