Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist to deliver patient-centered care in the home, reporting to the Clinical Manager. You will plan and administer prescribed PT treatments to improve function and independence, working with a team of clinicians to optimize patient outcomes.

The role requires a current physical therapy license, CPR certification, and collaborative experience. The position offers full-time hours and a pay range of $93,000 to $128,000 per year, with

Current and unrestricted Physical Therapy license.
Current CPR certification.
Experience collaborating with a team of healthcare professionals.

Responsibilities

Plan and administer prescribed PT treatment for patients in their homes to improve function.
Assess patients' physical strengths to guide treatment goals.
Develop and implement rehab programs aligned with physician plans and patient goals.
Provide PTAs and Home Health Aides with clear care instructions and oversee their work.
Document observations, interventions, and evaluations promptly.
Report progress to the physician, Clinical Manager, care team, patient, and family, and submit required documentation.
