Job summary

PathWell Health seeks a licensed Physical Therapist to provide in-home patient care across Connecticut. You will develop and adjust rehabilitation plans, evaluate function, and educate patients and families on exercises to improve mobility and reduce pain.

The role requires a PT degree and CT license, with preference for prior home health experience. Strong clinical, organizational, and communication skills are essential, with the ability to manage multiple cases and work well under pressure.

Qualifications

• Graduation from an approved Physical Therapy curriculum.
• Current and valid Connecticut Physical Therapist license.
• Recent home health experience preferred.
• Driver's license, car insurance, and car registration.
• Strong clinical assessment, organizational, and communication skills.
• Ability to handle multiple assignments simultaneously.
• Bilingual (Spanish) preferred.

• Ability to work well under pressure and stay calm and focused in emergent situations.

Responsibilities

• Provide direct patient care as prescribed, including evaluation and treatment planning.

Job Summary:

PathWell Health, a trusted and community-focused home health care provider, is seeking a highly skilled and compassionate Physical Therapist to join our team. As a Home Health Physical Therapist, you will work with patients in their homes to improve mobility, relieve pain, increase strength, and promote overall well-being.

Essential Duties and Responsibilities:

• Develop, implement, and adjust a conditioning/rehabilitation plan of care in partnership with patients, representatives, caregivers, and physicians
