Job summary

Pathwell Health is seeking a Physical Therapist to assess, plan, organize, and participate in rehabilitative programs aimed at improving mobility, reducing pain, and increasing strength. Relocation for candidates is encouraged and may be considered.

The role emphasizes collaboration with clients, caregivers, and physicians to develop and adjust care plans, with strong emphasis on documentation and direct patient care.

Qualifications

• Graduation from an approved Physical Therapy curriculum.
• Current and valid Connecticut Physical Therapist license is required.
• Experience in home health is preferred.
• Driver's License, Car Insurance, and Car Registration are required.
• Bilingual Spanish skills are preferred.

Responsibilities

• Develop, implement, and adjust a rehabilitation plan of care with the client, caregiver, and physician.
• Test and screen the client's physical strength to assist in evaluating function.
• Educate client and family on exercises and therapeutic procedures to continue at home.
• Document observations, interventions, and evaluations in the chart.
• Provide direct patient care as prescribed, including evaluation and treatment planning.
• Review referrals and medical records to determine required treatments.
• Record prognosis, treatment, and progress in the patient chart.
