Compassus in Alabama seeks a Home Health Physical Therapist to deliver coordinated care to patients across age groups. You will plan, implement, and evaluate patient care plans to restore or maintain well‑being, perform initial and ongoing assessments, and collaborate with the interdisciplinary team to ensure quality outcomes.

• Bachelor of Science in Physical Therapy required.
• Two years of experience as a Physical Therapist in an acute care or rehabilitation setting highly preferred.

Responsabilidades

• Provides care adhering to infection control measures.
• Attends in-services and participates in in-service programs.
• Accepts clinical assignments within education and competence.
• Monitors cases for third-party payor requirements.
• Communicates effectively with patient/family and care team.
• Performs initial and ongoing assessments including OASIS when appropriate.
• Prepares and revises Plan of Care with physician input.
• Plans and provides treatment for neuromuscular, neurological, and orthopedic disorders.
• Instructs on transfer techniques and safe use of equipment.
• Performs other duties as assigned.
