Job summary

Interim HealthCare of Queen Creek & Gilbert seeks a Home Health LPN to deliver compassionate, in-home nursing across Queen Creek and surrounding East Valley communities. You will perform assessments, develop care plans, and provide skilled nursing visits with autonomy and clinical support.

Per diem role with flexible scheduling, competitive pay, and a team that backs you up while you educate patients and families and maintain timely documentation.

Qualifications

• Active Arizona LPN license required.
• CPR certification required.
• Home health experience preferred (1 year).

Responsibilities

• Perform patient assessments and contribute to care planning.

What You'll Do

• Perform patient assessments and contribute to care planning


• Develop and update individualized plans of care aligned with physician orders and patient goals


• Deliver skilled nursing services in the home setting


• Educate patients and caregivers on conditions, treatments, medications, and self-care


• Provide preventive, restorative, and rehabilitative nursing interventions


• Monitor progress, identify changes in condition, and coordinate appropriate follow-up


• Maintain timely, accurate clinical documentation


• Collaborate with the interdisciplinary team to support continuity of care

What You Bring

• Graduate of an accredited nursing program with an active Arizona LPN license


• Minimum of 1 year of LPN experience in Home Health preferred


• Home health, wound care, or case management experience preferred


• Current CPR certification


• Valid driver's license, auto insurance, and reliable transportation


• Strong clinical judgment, communication, organization, and documentation skills
