Job summary

BAYADA Home Health Care seeks an experienced Physical Therapist (PT) for home health visits in Pennsylvania. This per diem, part-time or full-time role serves adult and geriatric clients across Northumberland County with flexible scheduling.

One year of PT experience and PA licensure are required. The PT will work within a multidisciplinary team to assess, plan, implement, and revise care plans, educate clients and families, and document care using an EMR on a touch-pad tablet.

Qualifications

• Active Pennsylvania Physical Therapist license.
• Minimum of one year of clinical PT experience.
• Graduation from a program approved by the American Physical Therapy Association (APTA) or CAAHEA.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational and problem-solving skills.

Responsibilities

• Make home visits to clients in designated territories.
• Perform mobility, strength, balance, gait and safety assessments.
• Implement wound care and protective interventions.
• Develop and implement individualized PT care plans including exercises and gait training.
• Collaborate with nursing and multidisciplinary team to revise care plans.
• Educate clients and families on rehabilitative exercises and safety.
• Document observations and interventions using EMR on a tablet.
