Job summary

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for full-time or part-time home health visits across York and Adams Counties, PA. The role requires PA licensure and at least one year of PT experience, with responsibilities spanning assessment, plan development, and care coordination in clients' homes.

You will work as part of a multidisciplinary team, delivering rehabilitative care in private homes and documenting progress using EMR/tablet.

Qualifications

• Current Pennsylvania PT license.
• Minimum of 1 year of PT experience.
• Graduation from a PT program approved by APTA/AMA.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform mobility, strength, balance, and safety assessments.
• Implement wound care using evidence-based dressings and topical agents.
• Develop and implement individualized care plans with manual therapy and gait training.
• Assess and revise the PT care plan with nursing in multidisciplinary team.
• Educate clients and families in rehabilitative care and activities.
• Document observations and interventions via tablet/EMR.
