Job summary

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for full time, part time, or per diem opportunities to perform home health visits in the Lansdale area. The PT will provide patient-centered care, assess needs, and develop plan of care for adult and geriatric clients in their homes.

Qualified candidates will hold a PA PT license, have at least one year of PT experience, and be proficient with EMR systems.

Qualifications

• PA Physical Therapist license required.
• Minimum of one year of PT experience.
• Graduate from an APTA-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements for mobility, balance, strength, and safety.
• Implement wound care using evidence-based dressings and topical agents.
• Develop and implement individualized care plans with manual therapy, gait training, and balance exercises.
• continually assess and revise the PT plan and collaborate with nursing in the care plan.
• Educate clients and family members on rehabilitative care and activities to promote health and independence.
• Accurately document observations, interventions, and evaluations in EMR.
