Job summary

Qualifications

• Licensed to practice as a physical therapist in NY.
• Strong verbal and written communication and organization skills.

Responsibilities

• Assess therapeutic, rehabilitative, and functional status of patients.
• Develop care plans tailored to patient needs.
• Evaluate home environments to support rehabilitation goals.
• Assess muscle strength, mobility, gait, ROM, and rehab potential.
• Instruct patients/caregivers in use and care of therapeutic appliances.
• Maintain detailed patient records and discharge plans.
