Qualifications

• Licensed to practice physical therapy in NY
• Degree from PT program approved by relevant associations
• Two years of PT experience preferred, home health experience a plus
• Strong verbal and written communication and organization skills

Responsibilities

• Assess patients' therapeutic and functional status in home setting
• Develop and implement individualized care plans
• Evaluate home environment for rehabilitation suitability
• Instruct patients/caregivers in use of therapeutic appliances
• Maintain accurate patient records and progress notes
