Job summary

Personal Touch Home Care is seeking a compassionate Physical Therapist for home visits in New York, NY. This fee-for-service, 1099 contractor role focuses on delivering high-quality care in patients’ homes under physician plans of care.

Responsibilities include evaluating patients, creating care plans, and educating patients and caregivers to promote mobility and independence. Licensing in NY is mandatory and travel within assigned areas is expected.

Qualifications

• Licensed Physical Therapist in NY.
• Graduation from a PT program approved by CAPTE or AMA allied health education bodies.
• Two years of PT experience preferred; home health experience preferred.
• Excellent verbal and written communication and organization skills.

Responsibilities

• Assess patients’ therapeutic, rehabilitative, and functional status at home.
• Develop and update individualized care plans.
• Evaluate home environments for safety and suitability for rehab.
• Assess strength, mobility, gait, ROM, and rehabilitation potential.
• Instruct patients and caregivers in use of therapeutic devices and exercises.
• Maintain detailed progress notes, assessments, and discharge planning.
