Job summary

PARS Therapy is seeking a compassionate Physical Therapist (PT) to join our dedicated home health team in Lake Jackson, TX. This onsite role focuses on helping patients regain movement, reduce pain, and improve quality of life in the comfort of their homes.

Qualifications

• Active Physical Therapist license in TX
• Professional liability insurance coverage in TX
• Graduate degree or higher in Physical Therapy from an accredited program
• Experience in home health or rehabilitative settings preferred
• Up-to-date knowledge of evidence-based treatment techniques and best practices
• Excellent communication skills and a friendly, professional demeanor

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting
• Develop and implement individualized treatment plans to restore function and mobility
• Provide interventions including exercise, gait training, manual therapy, and balance retraining
• Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies
• Monitor patient progress and adjust plans as necessary
• Document evaluations, treatment sessions, and progress notes using digital platforms
• Collaborate with physicians, nurses, and occupational therapists
• Conduct home safety assessments and recommend fall prevention measures
• Deliver compassionate care to empower patients toward independence
