Job summary

BAYADA Home Health Care is seeking a Physical Therapist (PT) for home health visits in the MUSC Health at Home by BAYADA network serving Florence/Marion Counties, South Carolina. The role supports patients with rehabilitative care and skilled nursing, with options for per diem, part-time, or full-time employment.

The PT will work as part of a multidisciplinary team, delivering care in patients' homes and documenting evaluations using electronic medical records.

Qualifications

• Licensed Physical Therapist (PT) in SC.
• At least 1 year of PT experience.
• Graduate of an APTA-approved PT program.
• Ability to work independently and manage time effectively.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility, strength, balance, and safety.
• Implement wound care using evidence-based dressings and topical agents.
• Develop and revise individualized care plans including exercises and gait training.
• Collaborate with nursing in the multidisciplinary care plan.
