Job summary

BAYADA Home Health Care is seeking a Physical Therapist to provide home health visits across Florence/Marion Counties. The PT will assess mobility, wounds, gait, and balance, and implement individualized care plans with therapeutic exercises and education for clients and families.

Qualified candidates hold a current PT license in SC, have at least one year of PT experience, and are proficient with EMR systems.

Qualifications

• A current SC Physical Therapist - PT license.
• Minimum of one year of PT experience.
• Graduation from a program approved by relevant PT boards.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated territory.
• Perform mobility, strength, balance, and gait assessments.
• Implement wound care and protective interventions.
• Develop and revise individualized care plans with PT interventions.
• Educate clients and families on rehabilitative care and activities.
• Document observations and interventions in EMR.
