Job summary

MUSC Health at Home by BAYADA is seeking a Per Diem Physical Therapist to provide home health visits for our Georgetown County, SC office. You will be part of a multi-disciplinary team delivering rehabilitative care to help clients remain at home.

The role requires a SC PT license and at least one year of PT experience, with the ability to work independently and document care using EMR. Collaboration with nursing and ongoing patient education are key aspects of this position.

Qualifications

• SC PT license required.
• Minimum of 1 year PT experience.
• Graduation from an APTA-approved program with transcript or diploma.
• Ability to work independently and manage time.
• Strong interpersonal and organizational skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in Georgetown County, SC.
• Perform mobility, ROM, strength, balance, and safety assessments.
• Implement wound care and therapeutic interventions.
• Develop and update individualized care plans.
• Educate clients and families on rehabilitative care.
• Document care using EMR and coordinate with the team.
