Job summary

Adaptive Home Health is seeking a Physical Therapist to provide comprehensive in-home care in a field-based setting. You will own the caseload, perform in-home assessments, and execute a clear plan of care focused on gait, transfers, strengthening, and home safety.

Qualifications

• Active Texas PT license (in good standing).
• Strong clinical judgment and comfort working independently in the field.
• Comfortable escalating changes in condition to RN/MD and documenting rationale clearly.
• Strong communication and documentation.
• Reliable follow-through on orders, frequencies, and patient goals.
• Current driver’s license and comfort with daily travel across assigned territory.
• Able to carry/transport basic therapy equipment as needed.
• Competency in core home health PT scope.

Responsibilities

• Own the caseload as a case manager: therapy plan, progression, and outcomes.
• Complete comprehensive in-home assessments and deliver skilled PT visits.
• Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.
• Set measurable goals, track progress, and adjust treatment if needed.
• Drive multidisciplinary coordination: communicate changes, align on orders, keep care team moving.
• Manage treatment and outcomes directly, including PTA supervision where appropriate.
• Coach patients and caregivers with practical home programs.
• Document promptly and thoroughly in Homecare Homebase (HCHB).
